Hospice care is a type of care that focuses on the quality of life for individuals who are experiencing advanced illness. Its goal is to provide comfort and support for the patient and their family, rather than a cure for the illness. Hospice care focuses on management of symptoms so the patients can continue the rest of their life in as little pain as possible.

Coming Home Hospice plans to treat all our patients like family. We promise to provide the best care regardless of the ability to pay. CHH plans to be lean on management and to focus more on having caregivers available to provide quality care. To learn more about Coming Home Hospice please check out our About page.

The hospice team is made up of healthcare professionals who work together to provide comprehensive care for the patient and their family. This may include a doctor, nurse, social worker, chaplain, and other specialists as needed.

Hospice care is available to individuals who have been diagnosed with a life-limiting illness and have a life expectancy of six months or less.  If you are not sure if you are eligible for hospice care, you can talk to your doctor or send us an email for a consultation.

Yes, hospice care can be provided to individuals who are still receiving treatment for their illness. Hospice care can be provided alongside curative treatments to help manage symptoms and provide emotional support.

We provide a range of services, including pain and symptom management, emotional and spiritual support, medication management, medical equipment and supplies, and bereavement support for the patient and their family. To learn more please check out our services offered page.
